Appointment policies
- We recommend an annual wellness visits annually for all age groups.
- Medication refills require appointments on a routine basis from every 3 to 6 months.
- Certain third-party forms may incur a fee for completion and/or require an appointment.
- Co-pays are due at the time of service; we do not bill for co- pays.
- $95 will be collected at the time of service for high deductible insurance plans.
- $25 will be charged for appointment cancellations with less than 24 hours notice and for no shows.
- $50 will be charged for second time no-shows and cancellations with less than 24 hours.
- Acceptable forms of payment include: cash, checks, American Express, Discover, MasterCard, Visa.

Cancellation Policy: A $25.00 fee will be charged when a patient fails to provide us with at least 24 hour notice of cancellation, or is a “No Show” for their appointment.

Unfortunately, we do not accept Medicaid or Kaiser Pernanente.
We accept a wide range of insurance plans, including Medicare.
